Dear Parents and Guardians,

We want to inform you that the State of Oklahoma has implemented new procedures for requesting vaccine exemptions for school attendance. Beginning this year, all exemption requests must be submitted online. Once approved, it is the parent or guardian’s responsibility to provide the exemption documentation directly to the school. Paper exemption forms will no longer be accepted.

Important: Previously approved paper exemptions remain valid and do not need to be resubmitted—unless your student is entering 7th grade. In that case, a new exemption must be submitted through the online system.
